Site-Specific Safety Orientation Forms

To gain access to certain workspaces, a site-specific orientation form must be filled out. The orientation form should be filled out during a guided tour of the space(s) with your supervisor or a designated representative, except for self-orientations. Please note, only handwritten submissions will be accepted.

Low-Risk Spaces

Teaching Spaces – for Instructional Staff

• Safety Orientation Form for Returning TAs for Classrooms
• Safety Orientation Form for Returning TAs for Laboratories
• The Safety Orientation for instructional Staff will be provided at the Orientation

Student Access (non-research) Spaces

• Course / cohort-specific forms are only available during your orientation.
• RH120 Student Team Safety Orientation Form. This must be submitted by your designated team representative
